For changing the name of someone who is at least 16 years of age.
15 year olds can apply but the Deed Poll document issued will be dated for their 16th birthday.
For changing the name of a child (who is under 16 years of age).
The applicant must have parental responsibility for the child.
For changing the surname of a family (i.e. at least one parent and one child who is under 16 years of age).
The parent(s) can also change their forenames if required and the child(ren) included will be added to the applicant parent’s Deed Poll document.
If you previously submitted your signed Deed Poll for safekeeping through our Legal Copy and Archive Service, we can provide additional certified copies from our records. The cost for this service is £14, which includes two legal copies. If you need more, extra copies are available for £2 each. For instance, if you require three legal copies, the total fee would be £16.
Please be aware that the initial £14 charge covers the search of our archives. If your Deed Poll is not found, you may opt for a replacement, in which case the £14 will be credited towards the cost of the new document. However, if you choose not to proceed with a replacement, the fee is non-refundable.
For Deed Polls issued before the year 2000, we do not hold archived copies, as our Legal Copy and Archive Service was introduced after that date.
Additionally, we can only release legal copies to the individual(s) named on the Deed Poll. If the document pertains to a child, they can request copies once they have reached at least 16 years of age.
